Medicare Advisor in Poway: Getting Your Prescriptions and Doctors Right
Poway calls itself “The City in the Country,” and its residents tend to be settled: long-time homeowners in Green Valley, Old Poway and the Garden Road area, many of whom retired from careers in engineering, defense, biotech or education. When they reach 65, they are usually leaving good employer coverage, and one of the first worries is prescriptions. Will the medications I take now still be covered, and what will they cost?
Brian McArthur, creator of the Medicare Execution Process®, works with Poway residents and their financial advisors to answer that alongside the bigger choice between a Supplement and Medicare Advantage. Our Escondido office is a short drive up I-15.
Health care in and around Poway
- Palomar Medical Center Poway: Poway’s own hospital, part of Palomar Health (formerly known as Pomerado Hospital).
- Palomar Medical Center Escondido: Palomar Health’s larger campus, just north.
- Scripps, Sharp and UC San Diego Health: to the south, used for specialty care and complex cases.
The hospitals above accept Original Medicare. Medicare Advantage networks vary by plan and year, so we check whether Palomar Health and your specialists are in network before you choose.
Turning 65 in Poway
- Initial Enrollment Period: the three months before, the month of, and the three months after your 65th birthday month.
- Retiring from a large employer: creditable coverage may let you delay Part B without penalty while you keep working. When it ends, you have a Special Enrollment Period to sign up.
- Drug coverage: keep creditable drug coverage or enroll in Part D on time to avoid a late enrollment penalty.
- Medigap open enrollment: six months from your Part B effective date, with no medical questions.
Part D: planning around your prescriptions
Each Part D plan, and each Medicare Advantage plan with drug coverage, has its own formulary, pharmacy network and pricing tiers. The same medication can cost very different amounts from one plan to the next. In 2026, total out-of-pocket drug costs in Part D are capped each year, which helps people on expensive medications, but premiums and coverage still vary. We enter your actual prescription list and preferred pharmacy and compare the plans we offer.
Original Medicare + Medigap vs. Medicare Advantage
- Original Medicare + Supplement: Part B at $202.90/month standard in 2026 (higher with IRMAA), plus a Plan G or Plan N premium and a separate Part D plan. You can see any provider who accepts Medicare. Plan G covers the gaps except the $283 Part B deductible.
- Medicare Advantage: one private plan with a network, prior authorization and an annual out-of-pocket maximum, usually with drug coverage included. It can fit someone whose doctors are all in one network.
Neither is automatically right. We compare them with your doctors and prescriptions in front of us.
IRMAA and your retirement income
Pensions, deferred compensation and RMDs can all push Poway retirees into IRMAA. 2026 Part B brackets for single filers (married filing jointly use double the thresholds):
- Up to $109,000: $202.90/month
- $109,001 – $137,000: $284.10/month
- $137,001 – $171,000: $405.80/month
- $171,001 – $205,000: $527.50/month
- $205,001 – $500,000: $649.20/month
- Above $500,000: $689.90/month
Part D carries its own IRMAA surcharge at the same income levels. Both are based on your tax return from two years earlier. Form SSA-44 can request a lower bracket after a life-changing event such as retirement or loss of pension income.
Working with your financial advisor
Brian coordinates with your financial advisor or CPA on retirement date, pension elections and withdrawal strategy, so Medicare costs are part of the plan rather than a surprise. Your advisor gets a summary of what was placed. There is no fee-sharing arrangement.
Frequently asked questions
Does Palomar Medical Center Poway accept Medicare?
It accepts Original Medicare. With Medicare Advantage, it depends on the plan’s network for that year.
How do I know my medications will be covered?
We compare Part D or Advantage drug coverage using your actual prescription list and pharmacy before you enroll.
I’m retiring from a large employer. When should I sign up for Part B?
Usually so that Part B starts the month your employer coverage ends. We help you time it and complete the paperwork.
Is there a penalty if I sign up for Part D late?
There can be, if you go 63 days or more without creditable drug coverage after your Initial Enrollment Period.
Do you only work with people who have a financial advisor?
No. Many clients come through advisors, but anyone in Poway is welcome to book a call.
