Medicare Advisor in Rancho Santa Fe: Why Original Medicare + Supplement Is the HNW Default Here
For most Rancho Santa Fe pre-retirees, the Medicare conversation ends in the same place: Original Medicare paired with a Medicare Supplement (Plan G or Plan N), not Medicare Advantage. The reason is direct — the physicians and hospital systems Rancho Santa Fe residents already use, primarily Scripps Health and UC San Diego Health, work cleanly with Original Medicare and unevenly (or not at all) with most Medicare Advantage plans available in San Diego County.
If you live in the Covenant or anywhere across 92067 / 92091 and you’ve spent twenty years building a relationship with a Scripps cardiologist or a UC San Diego oncologist, the conversation is short. The decision is mostly about which Supplement plan, not whether to use one.
The local network reality: where Rancho Santa Fe gets its tier-1 care
Rancho Santa Fe residents typically pull from four facilities for serious medical work:
- Scripps Memorial Hospital La Jolla — the closest full-service tier-1 hospital, ~15 minutes south on the 5
- Scripps Clinic Carmel Valley — outpatient and specialty care, the everyday workhorse for most Covenant residents
- Scripps Green Hospital — tertiary care, transplant, oncology
- UC San Diego Health (La Jolla / Hillcrest campuses) — academic medicine, specialty surgery, complex cases
All four accept Original Medicare without network parsing. Walk in with a Medicare card and a Medigap supplement, see any physician on staff, follow any referral, schedule any test. Most Medicare Advantage plans serving San Diego County either don’t include all of these systems or include only specific medical groups within them — and those rosters change between plan years.
This is the same logic that drives HNW patients to Mayo Clinic nationwide. Mayo only accepts Original Medicare. If you want guaranteed access, Original Medicare + Supplement is the door. Same play, scaled to North County.
The IRMAA reality at Rancho Santa Fe income levels
Most Rancho Santa Fe pre-retirees land in the top one or two IRMAA brackets. There’s no soft way to say it — at this income level, Medicare takes a pound of flesh.
The 2026 Part B numbers, single filer (married filing jointly is double the income thresholds, same per-person premium):
- $205,001 – $500,000: $649.20/month per person
- Above $500,000: $689.90/month per person
For a married couple in the top bracket, that’s $1,379.80/month combined just for Part B. Layer Part D IRMAA ($91/month at the top) and your Supplement premium on top, and the all-in monthly cost for two people approaches $1,800-$2,000 before any care is delivered.
That number is non-negotiable. The two-year lookback means your 2024 tax return determines your 2026 premium. If you retire and your income drops, you can file Form SSA-44 to request a reconsideration based on a life-changing event — which can accelerate the IRMAA relief without waiting two years for the tax cycle to catch up.
For HNW retirees with portfolio income, RMDs, deferred comp, and capital gains, IRMAA never goes to zero. We plan around it, not against it.
The concierge medicine question
Roughly half of new Rancho Santa Fe clients walk in with concierge primary care — an annual retainer relationship with a physician in La Jolla, Solana Beach, or Carmel Valley. The first question we ask: does your concierge doctor accept Medicare Advantage?
Most don’t. Concierge practices generally accept Original Medicare and bill the patient directly for the retainer. Medicare Advantage’s managed-care architecture — in-network primary care, prior authorizations, referral routing — doesn’t fit how concierge practices operate.
Once you put a number on what the concierge relationship is worth to you — the autonomy, the access, the absence of waiting room friction — Original Medicare + Supplement is almost always the obvious answer. The higher monthly premium isn’t a cost. It’s the price of keeping the medical life you’ve already built.
Plan G vs Plan N for Rancho Santa Fe HNW
Two Supplement plans matter for new enrollees: Plan G and Plan N.
Plan G covers everything Original Medicare doesn’t pay except the $283 annual Part B deductible (2026). After that deductible, your covered medical costs run to zero. No copays, no coinsurance, no surprise bills. Higher monthly premium, near-zero variability after January.
Plan N has the same architecture with slightly lower premium and small copays — up to $20 per office visit, up to $50 per ER visit (waived if admitted). Typically $25-40/month less than Plan G.
For most Rancho Santa Fe clients, Plan G wins on predictability. The premium difference between G and N is rounding error against a top-bracket IRMAA bill, and the “zero out-of-pocket after deductible” structure of Plan G is what HNW retirees actually want.

Do my UCSD Health specialists accept Medicare Advantage?
Some do, some don’t, and the lineup changes annually. UC San Diego Health participates in select Medicare Advantage networks, but not every physician within UCSD is available under every MA contract, and academic specialists in particular often restrict to Original Medicare. If you have an established UCSD specialist relationship you want to keep, Original Medicare + Supplement is the cleaner path.
I’m in the top IRMAA bracket. Does that change my plan options?
No — IRMAA affects what you pay for Part B and Part D, not which plans you’re eligible for. Top-bracket clients still choose between Original Medicare + Supplement and Medicare Advantage on the same terms as anyone else. What IRMAA does change is the math: when you’re already paying $689.90/month for Part B, the marginal cost of adding a Plan G Supplement is small relative to the network freedom it buys.
Can my financial advisor refer me directly?
Yes. Most Rancho Santa Fe clients are referred through their financial advisor, wealth manager, CPA, or estate planning attorney.
