Medicare Advisor in Vista: Keeping Your Doctors and Reviewing Coverage Each Year
Vista sits between the coast and the inland valleys, and its residents tend to have doctors in both directions. Someone in Shadowridge may see a primary care doctor near Tri-City and a specialist in Escondido. Someone in the hills off South Santa Fe Avenue or near Downtown Vista may have been with the same practice for decades. When Medicare comes up, the first question we hear in Vista is simple: can I keep the doctors I already have?
Brian McArthur, creator of the Medicare Execution Process®, helps Vista residents answer that before choosing a plan, and then checks the answer again every year. Our Escondido office is about 15 minutes east.
Hospitals Vista residents use
- Tri-City Medical Center: on Vista Way in Oceanside, near the Vista border. For many households it is the closest full-service hospital.
- Palomar Medical Center Escondido: the other main option, east along Highway 78.
- Scripps and UC San Diego Health: further south, used for specialty and complex care.
All of these accept Original Medicare. Under Medicare Advantage, which of them is in network depends on the plan, and networks can change at the start of any year. For Vista residents who split their care between Tri-City and Palomar, that is the detail worth checking first.
Turning 65 in Vista
- Initial Enrollment Period: the three months before your 65th birthday month, the month itself and the three months after.
- Part B timing: if you are still working with creditable employer coverage, you may be able to delay Part B without a penalty. Confirm it in writing before you rely on it.
- Medigap open enrollment: a six-month window starting with your Part B effective date, with no health questions.
Original Medicare + Medigap vs. Medicare Advantage
With Original Medicare plus a Supplement, you pay a monthly premium for predictable costs and the freedom to use any provider that accepts Medicare. In 2026, Part B is $202.90/month (more with IRMAA). Plan G covers the gaps except the $283 Part B deductible. Plan N costs less and adds small copays. You add a separate Part D drug plan.
With Medicare Advantage, a private plan replaces Parts A and B coverage with a network, prior authorization rules and an annual out-of-pocket maximum. It often includes drug coverage. It can fit someone whose doctors are all in one network and who is comfortable with that structure.
California’s birthday rule: a reason Vista clients review every year
California gives Supplement holders an advantage most states do not. Around your birthday each year, you can switch to another Supplement with equal or lesser benefits, without medical underwriting. Supplement premiums tend to rise over time, so we look at this with clients every year. Medicare Advantage plans are reviewed during the Annual Enrollment Period, October 15 to December 7, because networks, drug lists and costs can change on January 1.
IRMAA in brief
The 2026 Part B brackets for single filers are below (married couples filing jointly use double the thresholds). IRMAA is based on your tax return from two years earlier:
- Up to $109,000: $202.90/month
- $109,001 – $137,000: $284.10/month
- $137,001 – $171,000: $405.80/month
- $171,001 – $205,000: $527.50/month
- $205,001 – $500,000: $649.20/month
- Above $500,000: $689.90/month
If retirement or another life-changing event has lowered your income, Form SSA-44 can be used to request a lower bracket.
Working with your financial advisor
If you work with a financial advisor or CPA, Brian coordinates with them on retirement timing and income so IRMAA does not catch you off guard. They get a short summary of the coverage you chose. There is no fee-sharing arrangement.
Frequently asked questions
Is Tri-City Medical Center in Medicare Advantage networks?
It depends on the plan and the year. With Original Medicare and a Supplement, you can use Tri-City as you would any hospital that accepts Medicare.
My doctor is in Vista but my cardiologist is in Escondido. Which coverage keeps both?
Original Medicare with a Supplement lets you see any provider who accepts Medicare. With Medicare Advantage, we check whether both doctors are in the plan’s network before you enroll.
What is the California birthday rule?
It lets Supplement holders switch to another Supplement with equal or lesser benefits around their birthday each year, without health questions.
How often should I review my Medicare coverage?
At least once a year. We review Advantage plans each fall during Annual Enrollment and Supplements around your birthday.
Do you charge for the planning call?
No. There is no cost for the planning conversation.
