Medicare Advisor in Rancho Bernardo: Reviewing Your Coverage in Retirement

Rancho Bernardo was planned from the start as a place to settle down, and many people here did exactly that. Neighborhoods built for active adults, such as Oaks North and Seven Oaks, have been home to retirees for decades, and the wider community along I-15 is full of people who have been on Medicare for years, not months.

That changes the conversation. Plenty of Rancho Bernardo residents are not turning 65 at all. They are 72 or 80, they picked a plan a long time ago, and they want to know whether it still fits their doctors, their prescriptions and their budget. Brian McArthur, creator of the Medicare Execution Process®, helps Rancho Bernardo residents and their financial advisors review that coverage with real numbers. Our Escondido office is a short drive north on I-15.

Health care in and around Rancho Bernardo

  • Sharp Rees-Stealy Rancho Bernardo: a Sharp Rees-Stealy Medical Group clinic on West Bernardo Drive, used by many residents for primary and specialty care.
  • Palomar Medical Center Poway: the nearest hospital, just east in Poway, now operating under the Palomar UC San Diego Health name.
  • Palomar Medical Center Escondido: the larger Palomar campus, a few exits north.
  • Hospitals further south in San Diego: often where people go for complex surgery, cancer care or a second opinion.

Original Medicare is accepted by most doctors and hospitals across the county. With Medicare Advantage, what matters is the plan’s network for that year, and a medical group you have used for years may or may not be included. We check your actual doctors before anything changes.

A note on Sharp Health Plan for 2027

Sharp Health Plan has announced it is leaving the individual Medicare Advantage market at the end of 2026. If you are enrolled in one of its Medicare Advantage plans, you will need to choose other coverage for 2027. Your Sharp doctors are not going away; what changes is the plan that pays them. We explain your options, and the timelines that apply, in What to do if you’re losing your Medicare Advantage plan in San Diego for 2027.

Already on Medicare? When to review

  • Annual Enrollment Period (October 15 – December 7): change Medicare Advantage or Part D plans for the coming year.
  • Medicare Advantage Open Enrollment (January 1 – March 31): if you are in an Advantage plan, you can switch to another or return to Original Medicare once.
  • Special Enrollment Periods: a plan leaving the market, a move, or losing other coverage can open a window outside the usual dates.
  • Switching to a Supplement later: outside your initial Medigap open enrollment, carriers in California can usually ask health questions. There are some exceptions, including certain situations when a plan leaves the market, so it is worth checking before you assume the door is closed.

Original Medicare + Medigap vs. Medicare Advantage

Parts A and B come from the federal government. A Medigap Supplement, Part D drug plans and Medicare Advantage plans come from private insurance companies.

  • Part B: $202.90/month standard premium in 2026, higher with IRMAA
  • Plan G: pays what Original Medicare leaves behind, except the $283 Part B deductible
  • Plan N: a lower premium in exchange for small office and ER copays
  • Part D: a separate drug plan matched to your prescriptions

Medicare Advantage bundles hospital, medical and usually drug coverage into one plan with a network. It can suit someone who is comfortable with one medical group coordinating care. Premiums, benefits and networks vary by plan and change yearly, so we compare the specific plans we offer in San Diego County against your doctors and medications.

IRMAA in retirement

Retirement does not always end IRMAA. Required minimum distributions, a home sale or a large Roth conversion can push Rancho Bernardo retirees into a higher bracket. The 2026 Part B brackets for single filers are below. Married couples filing jointly use double the income thresholds:

  • Up to $109,000: $202.90/month
  • $109,001 – $137,000: $284.10/month
  • $137,001 – $171,000: $405.80/month
  • $171,001 – $205,000: $527.50/month
  • $205,001 – $500,000: $649.20/month
  • Above $500,000: $689.90/month

Your 2026 premium is based on your 2024 tax return. If your income dropped because of a life-changing event, such as retirement or the death of a spouse, Form SSA-44 lets you ask Social Security for a lower bracket.

Working with your financial advisor

When a financial advisor is involved, Brian works with them so your Medicare coverage fits the rest of the plan: RMDs, Roth conversions, and the IRMAA effect of a one-time sale. Your advisor gets a summary of what was placed and why. There is no fee-sharing arrangement.

Frequently asked questions

I’ve had the same plan for years. Do I need to review it?

Plans change their premiums, drug lists and networks every year, even if your letter looks the same. A short annual review against your current doctors and prescriptions is a good habit.

I’m in a Sharp Health Plan Medicare Advantage plan. What should I do?

Watch for the notice from the plan, then compare your 2027 options before your coverage ends. Our losing your plan guide walks through the steps, and we can review your choices with you.

Can I still see doctors at Sharp Rees-Stealy Rancho Bernardo?

With Original Medicare, if the doctor accepts Medicare. With Medicare Advantage, it depends on whether that medical group is in the plan’s network for the year.

I’m 78. Can I switch to a Supplement?

Possibly. Outside open enrollment, carriers may ask health questions, though some situations give you a guaranteed right to buy. We look at your circumstances before recommending a move.

Do I need to come to the office?

No. Most meetings happen by phone or video, and we are happy to include an adult child or your financial advisor. Our Escondido office is close by if you prefer to meet in person.
